Assumption of Risk and Waiver of Liability:I, the undersigned parent/guardian, hereby give permission for my minor child to participate in personal training sessions at Fitness Lounge Two LLC, aka Fitness Lounge. I acknowledge and understand that participating in physical activities involves inherent risks, including but not limited to muscle strain, injury, and other physical conditions. I assume full responsibility for any risks or injuries that may occur as a result of the minor's participation in these activities.

I, the undersigned, do hereby release and hold harmless Fitness Lounge Two LLC, its employees, contractors, and agents from any and all liability, claims, demands, or causes of action arising from the minor's participation in personal training sessions. This release includes, but is not limited to, any injury, illness, or damage to property sustained during or as a result of the minor’s participation.

Acknowledgment of Independent Personal Trainer:

I understand and acknowledge that the personal trainer providing services to my minor child is an independent contractor and is not an employee of Fitness Lounge Two LLC. I further understand that Fitness Lounge Two LLC is not responsible for the actions, behavior, or performance of the personal trainer, and that any issues or concerns regarding the personal trainer's services should be addressed directly with the trainer.

Waiver of Liability and Indemnification:

I, the undersigned Parent or Legal Guardian, hereby agree to release, discharge, and hold harmless Fitness Lounge Two LLC, including its subsidiaries, affiliates, owners, directors, officers, employees, agents, contractors, vendors, and volunteers (collectively referred to as "Fitness Lounge Two Parties") from any and all liability, claims, demands, causes of action, or damages of any kind arising out of or related to any injury, harm, or damage sustained by my minor child (the "Participant") in connection with their use of the facilities, equipment, services, programs, or activities within or outside the premises of Fitness Lounge Two LLC.

This includes, but is not limited to, injuries resulting from the negligent acts or omissions of Fitness Lounge Two LLC or the Fitness Lounge Two Parties, as well as those arising from my own negligent acts or omissions, the negligent acts or omissions of the Participant, other members, guests, visitors, or any other persons on the premises.

I, the undersigned, further agree to defend, indemnify, and hold Fitness Lounge Two LLC and the Fitness Lounge Two Parties harmless from and against any and all claims, demands, or actions brought by anyone, including but not limited to my minor child, related to any injuries, damages, or harms sustained by the Participant while using the facilities or participating in any activities at Fitness Lounge Two LLC.

My agreement to defend means that I will cover all reasonable legal fees, expenses, and costs incurred by Fitness Lounge Two LLC and the Fitness Lounge Two Parties to defend against such claims, starting from the date the claim is asserted. My agreement to indemnify and hold harmless means that I will pay any and all settlement amounts, judgments, damages, fees, or other costs incurred by Fitness Lounge Two LLC and the Fitness Lounge Two Parties in connection with resolving the claim, including but not limited to any litigation costs, attorney fees, and court costs.

Consent for Medical Treatment:

In the event that my minor child requires emergency medical treatment while participating in personal training at Fitness Lounge Two LLC, I hereby consent to the administration of necessary medical treatment by a qualified medical professional. I agree to assume full responsibility for any costs associated with such medical treatment.
